PRIVACY POLICY
This privacy policy explains how H-Align Soft Tissue Therapy uses and protects your personal data. H-Align Soft Tissue Therapy is committed to ensuring that your privacy is protected.
Information we may collect includes the following:
- Name, address, telephone number, email address for the purposes of contacting you regarding your appointments, your treatment and home care plan.
- Information on health and medical history to provide you with the best possible treatment. You can, of course, refuse to provide the information, but if you were to do that we would not be able to provide treatment.
- Details from each treatment will be collected for purposes of treatment history and rehabilitation progress
All clients have the following Individual Rights under the Data Protection act 2018;
Right of access – This will enable you to request the personal data we hold about you.
Right to rectification – This will enable you to have any of your personal data rectified if it is inaccurate or incomplete. You have the right to see what personal data of yours we hold, and you can also ask us to correct any factual errors.
Right to restrict processing – This will enable you to block or suppress processing of your personal data as well as preventing us from submitting your details to third party companies. We will never share your data with anyone who does not need access without your consent. Information shall only be shared with your permission to another healthcare provider whom both you and I have deemed vital to be included in your treatment. Any other healthcare worker requesting information shall not be given without clear permission from yourself.
Right to erasure – The right to erasure is also known as the right to be forgotten. This will enable you to request the deletion or removal of personal data where there is no compelling reason for us to continue holding it. We have a legal obligation to keep your records for 7 years after the last treatment. In the case of minors, records will be kept for 7 years after they reach the age of 18, but after this period you can ask us to delete your records if you wish. Otherwise, we will retain your records indefinitely in order that we can provide you with the best possible care should you need to see us at some future date.
Clients can exercise their Individual Rights at any time without charge. However, if requests are considered repetitive, unfounded or excessive a reasonable administration fee can be charged.
All reasonable care will be taken to safeguard the privacy of all client’s personal data by H-Align Soft Tissue Therapy. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and managerial procedures to secure and safeguard the information we collect.
Your records are stored
- on paper, in locked filing cabinets
- on computers, password protected and backed up regularly
We will not release your personal information to third parties unless we have your permission or are required by law to do so. Provided we have your consent, we may occasionally send you general health information in the form of articles, advice or newsletters. You may withdraw this consent at any time by contacting helen@h-align.com
H-Align Soft Tissue Therapy will not use, sell or transfer your personal information for anything other than mentioned above. You will not receive any direct marketing or promotions through any of our third parties.
